On Thursday 14th February 2019, our CardioWellness4Slough team attended the sixth annual Cardiovascular Disease Prevention Conference in Manchester, hosted by Public Health England (PHE). The focus of this year’s conference was on scaling up CVD prevention to improve service impact and health care outcomes.
Solutions 4 Health presented a poster on our Integrated Cardiac Disease Prevention Programme. This focused on our CardioWellness4Slough integrated service which was designed to provide and coordinate early help for residents. This is achieved through better and faster access to information, advice and guidance, using health risks assessments and onward referral to appropriate services. Through this service, results have shown that integrating lifestyle services is fundamental in improving health and wellbeing.
We were pleased to attend this conference, focusing on prevention in tackling cardiovasular disease. Our service aims to empower people who might be at a risk of developing heart disease by enabling and motivating them to address lifestyle issues such as smoking, achieving a healthy weight, increasing physical activity and drinking too much alcohol. The service assesses and screens individuals to identify risk factors which may result in an opportunity to be referred to a local lifestyle service.
