Solutions4Health are pleased to announce the launch of it’s new falls prevention service for Sandwell!
Falls in hospitals are the most commonly reported patient safety incident with more than 240,000 reported in acute hospitals and mental health trusts in England and Wales. Community exercise classes and postural stability instruction provide a targeted approach to adults who have experienced one or more falls and are at risk of further falls, ensuring a community approach to preventive falls management to those most in need.
The best outcomes for older people with social care needs and multiple long-term conditions are improved quality of life, and increased independence, choice, dignity and control. These can be achieved through coordinated care that is person-centred, and, also by enabling older people to have a voice in decisions made regarding the services that affect them, as well as addressing other issues that may impact on their quality of life, such as social isolation and support for independent living.
Our FallsFree4Life programme will offer free virtual and community based (once Covid-19 restrictions allow) assessments to identify those at risk at falls and then develop a physical activity and exercise programme to improve their strength and balance.
